The following is a list of documents and forms that must be turned into the office no later than May 1, 2024.
**Students WILL NOT be able to attend without a completed student file**
Please turn in the following documents and forms. For already enrolled students, all forms can be found, filled out, and submitted on the ChildPilot portal. If you do not have a ChildPilot portal, please download the required forms below and turn them into the office (forms in red can be downloaded). Thank you!
o Birth Certificate (a copy)
o Emergency Card (complete with Code Word)
o Health History (signed by doctor)
o Immunization or signed Statement of Exemption on state required form
o Verification of Policies and Procedures (please read them —> click HERE, write down any questions you may have, and sign the Verification form)
o Sunscreen (for Preschool & Pre-Kindergarten classes) OR Sunscreen (for Kindergarten class)
o Parent Survey “I Can DO”
